"put one's foot in one's mouth"だけでなく、"put one's foot in it"という言い方もあるようです。後者の場合だと、知らなければ、私の場合、"it"は前出の何かと勘違いしそうです。1798年が記録に残っている最初だとすると、最近できた言い回しではないようです。
『Scholastic Dictionary of Idioms』には、その語源が載っていました。

This saying was first used in the late 1800s and early 1900s, and may have been suggested by "foot-and-mouth disease," a serious infectious sickness that affects cattle, sheep, pigs, and goats. But today this expression has nothing to do with a disease. A baby can easily put her foot into her mouth, but older people can't do it unless they're contortionists. It's a good thing, too, because they'd look pretty ridiculous. We use this expression when someone makes a verbal blunder by saying what he should not say just when he should not say it. So if you tell your friend that the cake had made you sick without knowing that he had baked it, you "put your foot in your mouth." Of course, if your foot were really in your mouth, you couldn't say the foolish thing to begin with, and that would be good.

1800年代後半から1900年代前半においては、口蹄疫(こうていえき)という難しい名前の極めて伝染力の高い家畜の病気に使われていたことばのようです。
『ウィキペディア』によると口蹄疫は、口や蹄に疱疹ができるようなので、"foot-and-mouth"なのでしょうか。

一般的には、感染すると発熱、元気消失、多量のよだれなどがみられ、舌や口中、蹄の付け根などの部位に水疱が形成され、それが破れて傷口になる。

しかし、現代の"put one's foot in one's mouth"と口蹄疫とはまったく関連ないようです。どこからどう変化したのか知りたいところですが、それは載っていません。ただ、たしかに、足を口に入れている姿は滑稽で「へま」をしたイメージと合いますし、実際に足が口に入っていたら、失言をする可能性はありません。そういう意味では、失言をしないように足ででも口を塞いでいたほうがいいことを指すのでしょうか。あくまでも想像でしかありませんが。
